Hospice RN Case Manager Job Description

Job Summary:

We are seeking a compassionate and skilled Registered Nurse to join our team as a Hospice RN Case Manager. As a Hospice RN Case Manager, you will be responsible for providing exceptional hospice care to patients and families, while upholding our high standards of nursing practice.

Key Responsibilities:

  • Maintain a specified case management load, ensuring timely and effective care for patients and families.
  • Conduct initial and ongoing assessments of patients' physical and psychosocial status, identifying needs and developing personalized care plans.
  • Collaborate with the Interdisciplinary Group (IDG) to develop and implement comprehensive care plans, ensuring seamless coordination of services.
  • Coordinate care among IDG members and other healthcare providers, ensuring continuous assessment and implementation of care plans.
  • Direct and supervise LPN/LVN, Hospice Aide, and Homemaker services, ensuring high-quality care delivery.
  • Evaluate and coordinate the need for services, including inpatient, respite, and continuous care, as indicated by patients' status and needs.

Requirements:

  • Graduate of an accredited school of nursing.
  • Licensed to practice as a RN in the state of South Carolina, with an active and good-standing license.
  • Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years, or evidence of current clinical knowledge.
  • One (1) year of hospice care experience, with precepting or training experience preferred.
  • CPR certification and valid driver's license with auto liability insurance.
