About the Role

VNS Health is seeking a highly skilled and experienced Registered Nurse to lead our Hospice Team as a Manager. As a key member of our team, you will be responsible for managing, coordinating, and facilitating the delivery of high-quality end-of-life care to patients and their families in the comfort of their own homes.

Key Responsibilities

  • Lead a dedicated interdisciplinary team to provide comprehensive hospice care services, ensuring the highest level of patient satisfaction and quality of care.
  • Develop and implement clinical and quality improvement initiatives to ensure compliance with regulatory requirements and achieve exceptional patient outcomes.
  • Collaborate with the Quality Improvement and Education teams to ensure professional practice standards are consistently met for each member of the team.
  • Manage the day-to-day operations of the department, including effective caseload or visit management, achievement of productivity and revenue/expenditure targets.
  • Participate in Hospice Team meetings, Interdisciplinary group management meetings, family meetings, and patient calls to ensure seamless communication and coordination of care.

Requirements

  • License and current registration to practice as a Registered Professional Nurse in New York State, required.
  • Bachelor's degree in nursing from an approved program accredited by the National League of Nursing or Health or Human Services field or the equivalent work experience, required.
  • Minimum three years of clinical nursing experience, preferably in a home care or hospice setting, required.
  • Clinical management experience, preferred.
