About the Role

We are seeking a Full-time, Hospice Registered Nurse Case Manager to join our team in Hudson / Bergen County, NJ. As a key member of our interdisciplinary team, you will work under the guidance of our Director of Nursing, attending physicians, and the interdisciplinary group to provide skilled nursing assessments, planning, and care, maximizing patient comfort and health.

Responsibilities:

  • Assess and respond to patient and loved ones/caregivers' needs in varied settings.
  • Provide supportive care in accordance with attending physicians' orders and plans of care.
  • Initiate communication with attending physicians, Hospice staff members, and other agencies as needed to coordinate optimal care and resource utilization.
  • Maintain regular communication with attending physicians regarding patient and loved ones/caregivers' needs.
  • Seek input from other team members to enhance patient care and support.
  • Document patient care accurately and timely in electronic medical records.
  • Participate in and contribute to Interdisciplinary Group meetings.
  • Coordinate physical care of patients by teaching primary caregivers, volunteers, or employed caregivers, or providing direct care as needed.
  • Meet regularly with Hospice nursing staff to review problems, share professional support, and exchange feedback.
  • Attend patient care conferences and facilitate discussion of issues from caseloads.
  • Supervise and document NACs as assigned.
  • Inform the Patient Care Coordinator of unusual or potentially problematic patient/family issues.
  • Assume the role of associate nurse when responding to patient/loved ones/caregivers of other primary nurses during on-call time or as requested.
  • Provide support at time of death and period of bereavement.
  • Participate in Hospice orientation and in-service training programs for professional staff.
  • Participate in agency and community programs to promote professional growth and understanding of hospice care.
  • Maintain familiarity with agency policies and State and Federal regulations governing hospice care.
About You

We seek a candidate with:

  • A current license to practice as a registered nurse in the State of NJ.
  • At least two years of work experience as a professional nurse, preferably in hospice care.
  • Ability to assess and respond to patient and loved ones/caregivers' needs in varied settings.
  • Skilled in nursing practice, able to cope with loved ones/caregivers' emotional stress, and tolerant of individual lifestyles.
  • Ability to make meaningful observations, skilled assessments, and write comprehensive, accurate reports.
  • A team player who supports an interdisciplinary team concept of caring for patients and loved ones/caregivers.
  • Fluency in both English and Spanish.
  • A valid driver's license, good driving record, reliable vehicle, and ability to drive year-round to locations.
  • Ability to take direction, work independently, and effectively as part of a multidisciplinary team in a positive, professional manner.
