Job Summary

We are seeking a highly skilled and compassionate Occupational Therapist to join our team at InnovAge. Key Responsibilities
  • Conduct comprehensive occupational therapy evaluations and assessments to identify participants' needs and develop personalized treatment plans.
  • Provide occupational therapy services, including exercises, activities, and education, to help participants achieve their goals and maintain their functional abilities.
  • Collaborate with our interdisciplinary team to develop and implement Plans of Care that meet the unique needs of each participant.
  • Communicate effectively with participants, family members, and healthcare professionals to ensure seamless care coordination and optimal outcomes.
  • Document all services provided, including progress notes, treatment plans, and participant statistics, in accordance with InnovAge's documentation requirements.
  • Participate in peer review and quality improvement activities to ensure the highest level of care and service delivery.
  • Adhere to InnovAge's policies, procedures, and standards, and maintain a clean and organized therapy department.
Requirements
  • Graduate of an Occupational Therapy curriculum accredited by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association.
  • National Certification by the American Occupational Therapy Association or eligible for National Registration Examination of the American Occupational Therapy Association.
  • Registration with the state in which practicing.
  • A minimum of one year of experience working with the frail or elderly is required.
  • Two (2) years' experience, preferred.
