Job Summary:

As a Hospice Aide, you will provide direct patient care and support to our patients in their homes. You will work closely with our interdisciplinary team to ensure that our patients receive the highest level of care and comfort.

Responsibilities:
  • Provide direct patient care, including personal care and hygiene assistance
  • Document patient care and progress in a timely and accurate manner
  • Collaborate with the healthcare team to develop and implement patient care plans
  • Provide emotional support and comfort to patients and their families
Requirements:
  • High school diploma or equivalent
  • Current listing with the Division of Facility Services Registry
  • One year of patient care experience in an acute care or custodial setting
  • Documented proof of at least 75 certified education hours
  • Ability to work independently and as part of a team
