Liberty Health System is seeking a compassionate and skilled Hospice Aide (CNA) to join our team. As a Hospice Aide, you will provide direct patient care and support to individuals with terminal illnesses, ensuring their comfort, dignity, and well-being.
Key Responsibilities:- Provide high-quality patient care, including personal care, hygiene, and mobility assistance.
- Document patient care and progress accurately and thoroughly.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans.
- Provide emotional support and companionship to patients and their families.
- High school diploma or equivalent, with successful completion of a Board of Nursing-approved training and competency evaluation program.
- Current listing with the Division of Facility Services Registry.
- One year of patient care and record-keeping experience in an acute care or custodial setting.
- Documented proof of at least 75 certified education hours.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
