Hospice Care Specialist

Liberty Health is seeking a compassionate and skilled Hospice Aide to provide high-quality patient care and support to individuals facing terminal illness.
Key Responsibilities:
- Deliver direct patient care services, including personal care and assistance with daily activities.
- Document patient care and progress accurately and thoroughly.
- Collaborate with the healthcare team to ensure comprehensive care and support.
Requirements:
- High school diploma or equivalent, with completion of a Board of Nursing-approved training program.
- Current listing with the Division of Facility Services Registry.
- Minimum one year of patient care experience in an acute care or custodial setting.
- Documented proof of at least 75 certified education hours.
- Ability to work independently and provide effective health care services to homebound patients.
Working Conditions:
- Accept travel and working conditions as assigned.
- Working knowledge of safe and appropriate personal care practices.
- CPR certification.
