Job Opportunity

We are seeking an experienced Medical Social Worker to join our Hospice team in Rock Hill, SC.

  • Provide social work services in accordance with the physician's plan of treatment.
  • Collaborate with the interdisciplinary team to assess and support patients and families.
  • Assess and provide bereavement counseling for patients and families.
  • Serve as a liaison with community resources.

Requirements

  • Master's or Bachelor's degree in social work from a Council of Social Work Education accredited or approved school.
  • Current licensure to practice as a social worker in SC.
  • Minimum one-year Hospice experience preferred.
  • Valid Driver's License.
