Job Summary

The Case Manager provides direct management services, including identifying and coordinating community resources, to support clients in achieving their short-term and long-term goals. This role requires strong communication and interpersonal skills, as well as the ability to work effectively under pressure.

Key Responsibilities
  • Interview clients to assess their direct service needs and develop a case plan with written short-term and long-term goals and tasks.
  • Make referrals as needed and reevaluate goals with clients at regular intervals.
  • Facilitate individual and group meetings as required and serve as a member of the multidisciplinary case team.
  • Collaborate with other disciplines, including substance abuse, legal, medical, educational, vocational, and mental health professionals, to ensure comprehensive support for clients.
  • Create and maintain accurate and up-to-date case records, including legal documentation, assessments, case notes, case plans, service updates, and discharge summaries.
  • Enhance the client environment by creating a welcoming and customer-oriented facility.
  • Enter DAP notes within 48 hours into the Homeless to Housing portal and conduct home visits/community visits for Housing Navigation and Housing Stabilization clients.
  • Complete reports and maintain statistical data, meeting programmatic, documentation, and performance standards on an ongoing basis.
Requirements
  • Bachelor's degree in human services or a related field.
  • Social work degrees require social work licensure in the appropriate jurisdiction.
  • Professional counseling degrees require professional counseling licensure in the appropriate jurisdiction.
  • One year of experience working with at-risk clients in a direct service setting.
  • Two years of experience working with clients who have a history of homelessness, with some knowledge of addiction and mental health issues preferred.
