Job Title: Case Manager/Social Worker

CMT Services, Inc. is seeking a highly skilled Case Manager/Social Worker to join our team. As a key member of our Family Services Division, you will be responsible for providing case management services to children, youth, and their families referred through various agencies.

Key Responsibilities:
  • Coordinate and provide services that are safe, timely, effective, efficient, equitable, and client-centered.
  • Solicit, screen, and accept referrals from agencies such as the Department of Social Services, Department of Family Services, and the Local Care Team.
  • Provide case management services to children, youth, and their families, including conducting home visits, completing intake assessments, and providing crisis intervention.
  • Collaborate with community resources and services providers to ensure comprehensive support for clients.
  • Provide administrative support to the Children, Youth, and Families Division, including data entry, report preparation, and correspondence.
Requirements:
  • Bachelor's Degree in a related field, such as Human Services, Sociology, or Psychology.
  • Excellent time management and organizational skills, with the ability to work under pressure.
  • Strong written and verbal communication skills, with the ability to effectively communicate with clients, colleagues, and community partners.
  • Proficient in Microsoft Office and other relevant computer programs.
  • Ability to work in a fast-paced environment and adapt to changing priorities.
