Job Summary:

Oak Ridge Rehabilitation and Healthcare is seeking a skilled Nurse Assessment Coordinator to join our team. As a key member of our nursing leadership team, you will be responsible for ensuring accurate and timely clinical documentation, maximizing company reimbursement, and providing exceptional patient-centered care.

Key Responsibilities:

  • Ensure accuracy of all sections of Multiple Data Set (MDS) coding to maximize company reimbursement consistent with the levels of treatment delivered and required for appropriate patient/resident care.
  • Ensure accuracy of clinical paperwork (assessment and accompanying documentation) to maximize company reimbursement consistent with the levels of treatment delivered and required for appropriate patient/resident care.
  • Nursing Educator: Serve as a primary trainer to the nursing department to ensure documentation is accurate, timely, and thorough.
  • Nurse Management: Serves as a key member of the nursing leadership team to clinically manage the unit in order to maximize patient-centered outcomes.
  • Case management with managed care to maximize therapy and nursing services provided to residents in order to maximize patient-centered outcomes.
  • Manage clinical and financial record keeping and dissemination of these records to appropriate payers.
  • Take joint ownership of care conferences with social workers.

Requirements:

  • Active Pennsylvania RN License.

Education: Graduate of an accredited nursing school.

Experience: Previous long-term care experience in MDS.
