Job Summary: Home Care Wellness LLC is seeking a skilled Per Diem Speech Therapist to provide one-on-one care to patients in their homes. As a Speech-Language Pathologist, you will help patients manage communication and language difficulties, swallowing problems, and cognitive disorders.

Responsibilities:

  • Evaluations: Assess patients' needs, including medical, psychosocial, and environmental needs. Evaluate speech, language, and swallowing difficulty levels.
  • Treatment Plans: Develop and implement individualized treatment plans with the patient and physician. Address specific functional needs, such as improving pronunciation, strengthening speech muscles, and learning to speak correctly.
  • Therapy: Help patients with speech, language, augmentative communication, and feeding. Conduct oral, pharyngeal, and motor breathing exercises, and teach clients how to make sounds and improve their voices.
  • Goal Setting: Establish realistic, measurable goals for patients to achieve and evaluate their progress.
  • Communication: Help patients communicate verbally and nonverbally, and overcome cognitive disorders that affect their ability to communicate. Counsel clients and their families on how to cope with communication and swallowing disorders.
  • Collaboration: Work with other members of the patient's care team, such as registered dieticians, medical social workers, physical therapists, occupational therapists, and homecare nurses. Communicate with doctors, teachers, and specialists to coordinate care.
  • Documentation: Complete paperwork and update charts. Documentation requirements may vary by agency, but each patient encounter is documented. Visit notes describe patient status, treatment, and response, teaching and response, assessment of the visit, and plans for future visits.

Qualifications:

  • Education: Master's or doctoral degree from an accredited program in communicative sciences and disorders. The program must be accredited by the American Speech-Language-Hearing Association's (ASHA) Council on Academic Accreditation in Audiology and Speech-Language Pathology (CAA) or another accrediting agency recognized by the US Department of Education.
  • Clinical Experience: At least one year of clinical experience in speech-language pathology. You may also need to complete 1,260 hours of supervised postgraduate clinical experience.
  • Licenses and Certifications: Certified to practice speech-language pathology and have a valid CPR certification. You may also need to have a current ASHA certification.
  • Other Requirements: Valid driver's license and an insured automobile.

Compensation: $42 - $55 hourly
