Job Summary

The Clinical Data Coordinator is responsible for ensuring the timely and accurate completion of resident assessments and care plans that meet federal and state guidelines. This role will coordinate the Resident Assessment Instrument (RAI) process, ensuring the timeliness and completeness of the Minimum Data Set (MDS), Care Area Assessments (CAAs), and interdisciplinary care plans.

Key Responsibilities
  • RAI Process Management: Ensure that the RAI and supporting documentation meet federal and state standards for regulatory and reimbursement purposes.
  • Training and Education: Participate in the training and education of interdisciplinary team members on the RAI process and quality measures.
  • MDS and Care Plan Completion: Complete initial, quarterly, annual, significant change, and special MDS and care plans in accordance with state and federal guidelines.
  • Electronic Submission: Ensure the timely electronic submission of all MDS to the facility's chosen scrubber and iQIES.
  • Accurate Reporting: Accurately report resident conditions and services provided in accordance with federal and state guidelines to maximize quality measure ratings and reimbursement.
  • CMI Maximization: Understand and maximize Case Mix Index (CMI).
  • Staff Education: Educate skilled nursing staff on MDS processes and documentation requirements.
  • Interdisciplinary Collaboration: Partner with departments, including therapy and finance, to ensure accurate and timely billing for areas related to the MDS.
  • Medical Record Review: Perform medical record reviews as requested by CMS/HMOs using their guidelines.
  • Care Plan Scheduling: Schedule and conduct weekly care plans.
  • Interdisciplinary Team Meetings: Attend interdisciplinary team meetings as appropriate.
  • Care Pathway Development: Develop an appropriate care pathway and timely communication of needed information to the resident, family, other healthcare professionals, and third-party payers.
  • Assessment Reasoning: Select the correct reason for assessment and Assessment Reference Date (ARD).
  • State Validation Reports: Review state validation reports and ensure appropriate follow-up action is taken.
  • MDS Scheduling: Schedule MDS to maximize reimbursement.
  • MDS Coding: Complete accurate coding of the MDS with information obtained via medical record review, observation, and interview with facility staff, resident, and family members.
  • Facility Policies: Follow Seneca Hill Manor facility policies and procedures as they pertain to specific duties.
  • MDS and Care Conference Scheduling: Maintain MDS and care conference schedules.
  • Assigned Assessments: Complete assigned assessments to facilitate MDS completion.
  • Hospital Record Review: Check hospital records and MD/NP documentation on website for pertinent information regarding MDS.
  • Continuous Exposure: Continuously expose to patients, families, and the general public in addition to a general office environment.
Requirements
  • Supervisory Experience: Ability to supervise, direct, and evaluate nursing staff in the provision of resident care and performance of job duties.
  • Effective Communication: Demonstrated effective communication, conflict resolution, and problem-solving skills.
  • Knowledge of Aging Process: Knowledge of the process of aging as it relates to healthcare, as well as resident rights and abuse regulations.
  • Nursing Staff Management: Knowledge in determining the type and amount of nursing staff and supplies needed to provide quality resident care.
  • Nursing Equipment Use: Demonstrate correct and safe technique in the use of all nursing/resident equipment.
  • Communication Skills: Ability to communicate effectively, both orally and in writing.
  • Professionalism: Professionalism and business acumen appropriate for working with guests, clients, physicians, employees, and volunteers.
  • Time Management: Excellent time management skills, punctuality, and a sense of urgency in completing tasks.
  • Resource Management: Ability to cost-effectively manage resources and supplies.
Education and Experience
  • Licensure: Must have a current license to practice as a Registered Nurse (RN) in New York State.
  • Experience: One (1) year of skilled nursing facility, long-term care experience, rehabilitation nursing, or other healthcare-related facility experience, and previous experience in completing Minimum Data Set (MDS), Resident Assessment Instrument (RAI) process, and/or care plans.
  • Certification: Maintain current RAC-CT certification.
