We are seeking a skilled RN Care Coordinator to join our team at the University of Florida Health. The successful candidate will provide coordination of care and patient education for Maternal Fetal Medicine patients in the in-patient and out-patient setting.
Key Responsibilities- Arrange for appointments and facilitate authorization for patients referred to the MFM service.
- Triage calls for Maternal Fetal Medicine patients and document calls in Epic.
- Coordinate care of MFM current and new obstetrical patients, including tracking lab, pathology, cytology, and radiology procedures.
- Collect and assess patient data to determine patient needs and promote optimal care.
- Coordinate transfer of patient records and follow up with patients after surgery.
- Provide complete interdisciplinary management of nursing services for individuals seen by the Maternal Fetal Medicine providers.
- Coordinate education and guidance for patients and significant others to maximize achievement of expected outcomes.
- Minimum of three years nursing experience required.
- Up to 1 year experience in an ambulatory care setting in the field of Obstetrics and Gynecology preferred.
- Knowledge of regulatory agency standards, Nursing Standards, and Florida Nurse Practice Act required.
- Associate's degree in nursing is required.
- Bachelor's degree in nursing preferred.
- Registered Nurse (RN) - Florida Licensure required at time of hire.
- Basic Life Support (BLS) required at time of hire.
