Care Coordinator Job Description

We are seeking a highly skilled Care Coordinator to join our team at HealthcareSource Medical Center. As a Care Coordinator, you will play a vital role in ensuring the smooth delivery of patient care services.

Key Responsibilities:

  • Collaborate with field staff to provide exceptional patient care
  • Monitor adherence to Federal and State regulations
  • Ensure therapy staff has necessary resources to care for patients
  • Provide occasional day-to-day telephone support and coverage for field nurses

Requirements:

  • Registered Nurse or Physical Therapist degree required
  • 1-2 years acute care experience and 1 year previous home care experience preferred
  • ICD-9 coding experience a plus
