About the Role

We are seeking a highly skilled Clinical Pharmacist II to join our team at Yale New Haven Health. As a key member of our pharmacy department, you will play a critical role in providing exceptional patient care and contributing to the success of our organization.

Responsibilities
  • Customer Service Quality
    • Demonstrate and promote CHAMP behaviors, ensuring a positive and respectful experience for patients, families, and colleagues.
  • Medication Management
    • Conduct timely and accurate medication order reviews, considering patient characteristics, laboratory values, and vital signs.
  • Medication Preparation and Distribution
    • Demonstrate competency in pharmacy automation and technology, ensuring efficient and safe medication preparation and distribution.
  • Medication Safety, Regulatory Affairs, and Quality Assurance
    • Adhere to all departmental policies, procedures, protocols, and guidelines, ensuring a culture of safety and quality.
  • Leadership
    • Provide education and training to pharmacy staff, students, and residents, promoting knowledge sharing and professional growth.
  • Financial/Volume Results
    • Contribute to departmental cost-reduction and revenue generation initiatives, driving business success.
Qualifications
  • Education
    • Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree.
  • Experience
    • Completion of an ASHP-accredited PGY1 pharmacy residency or equivalent experience is preferred, but not required. Minimum of two years of relevant experience specific to practice area is required.
  • Licensure
    • Current Connecticut Pharmacist License, or eligible for pharmacist licensure in the state of Connecticut.
  • Special Skills
    • In-depth knowledge of pharmacology and therapeutics, and understanding of pharmacy systems of drug distribution and formulary management.
