We are seeking a highly skilled Clinical Pharmacist II to join our team at Yale New Haven Health. As a Clinical Pharmacist II, you will play a critical role in providing exceptional patient care and contributing to the success of our Ambulatory Care team.
Key Responsibilities- Customer Service Quality
- Demonstrate and promote CHAMP behaviors to ensure exceptional patient satisfaction and experience.
- Medication Management
- Conduct timely and accurate medication order reviews, considering all patient characteristics, including laboratory values and vital signs.
- Medication Preparation and Distribution
- Demonstrate competency in pharmacy automation and technology, as applicable to the work area.
- Medication Safety, Regulatory Affairs, and Quality Assurance
- Adhere to all departmental policies, procedures, protocols, and guidelines to ensure a safe and compliant environment.
- Leadership
- Provide education and training to other pharmacy staff members, pharmacy students and residents, and healthcare team members, via educational inservice or as a preceptor.
- Financial/Volume Results
- Contribute to departmental cost-reduction and revenue generation initiatives.
- Education
- Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree.
- Experience
- Completion of an ASHP-accredited PGY1 pharmacy residency or equivalent experience is preferred, but not required. Minimum of two years of relevant experience specific to practice area is required.
- Licensure
- Current Connecticut Pharmacist License, or eligible for pharmacist licensure in the state of Connecticut.
- Special Skills
- In-depth knowledge of pharmacology and therapeutics, and understanding of pharmacy systems of drug distribution and formulary management.
- Excellent verbal and written communication skills and the ability to work with others.
