Care Coordinator Opportunity

We are seeking a skilled Care Coordinator to join our team at UNC Health Care. As a key member of our acute care case management team, you will be responsible for coordinating patient care activities and collaborating with interdisciplinary teams to develop and implement effective care plans.

Key Responsibilities:

  • Assess patient medical, cognitive, psychosocial, and financial status
  • Identify patient problems and needs, as well as their strengths and barriers to care
  • Develop and maintain accurate and comprehensive patient records
  • Collaborate with patients, families, and caregivers to provide education and support
  • Prioritize caseload based on need and available resources

About the Role:

This is a full-time position working 40 hours per week, Monday through Friday. The ideal candidate will have a Master's degree in Social Work and experience working in behavioral health or social services. Compensation:

The estimated salary range for this position is $65,000 - $85,000 per year, depending on experience. We offer a competitive benefits package, including medical, dental, and vision insurance, as well as a 401(k) retirement plan.
