About the Job

As a nurse care manager, you'll work closely with our multidisciplinary team to develop and implement personalized care plans that address the unique needs of each patient. Your expertise and compassion will make a significant difference in the lives of our patients and their loved ones.

Key Responsibilities
  • You'll coordinate and deliver patient care in a fast-paced hospital setting, ensuring seamless transitions between care teams.
  • Documenting patient information, including care plans, progress notes, and discharge instructions, is a critical aspect of this role.
  • Providing exceptional clinical care that meets or exceeds our quality standards, always prioritizing patient safety and satisfaction.
  • Working collaboratively with your colleagues to share knowledge, best practices, and innovative solutions to improve patient outcomes.
  • Analyzing complex patient data to identify areas for improvement and developing strategies to enhance the overall care experience.
Requirements and Qualifications
  • You must be a licensed Registered Nurse in North Carolina or one of the compact states.
  • A BLS certification is a requirement for this role.
  • PREFERRED: Having one year of experience as a registered nurse in an acute care/inpatient hospital setting can be beneficial.
Additional Information

Please note that some units may require ACLS certification before performing clinical care, with training and certification opportunities provided by the employer.

Job Details

This is a full-time position with 36 hours per week, working night shifts and on-site. The job is exempt from overtime pay, and we offer a competitive salary package that includes a base salary range of $65,000 - $90,000 per year, depending on experience.
