Hospice Nurse

Abundant Staffing is seeking a compassionate and skilled Hospice RN to join our team. As a Hospice RN, you will provide high-quality patient care and coordinate with the interdisciplinary team to ensure the best possible outcomes for our patients and their families.
Responsibilities:
Conduct initial and ongoing comprehensive assessments of patients' physical, functional, psychosocial, and environmental needs
Develop and implement individualized plans of care (POCs) and recommend revisions as necessary
Educate patients and their families on disease processes, dietary needs, self-care techniques, and end-of-life care
Collaborate with the interdisciplinary team to prioritize patient needs and coordinate services
Use a case management approach to refer patients to other services as needed
Requirements:
Current state license as a Registered Nurse
Two years' experience as a Registered Nurse in a clinical care setting, preferably in hospice or home health
Excellent interpersonal skills and ability to communicate effectively
Proof of Hepatitis consent/declination

Physical and Mental Effort:
Prolonged sitting and walking required, with ability to lift up to 50 lbs. and move patients
Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition
Meet patient/family individualized psycho-social needs
