Job Summary

We are seeking a passionate Licensed Practical Nurse (LPN) to join our team of healthcare professionals. As an LPN with Elara Non-PCS, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities
  • Provide high-quality patient care in the home setting, including administering medications, wound care, and vital sign monitoring.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop individualized care plans to meet those needs.
  • Collaborate with other healthcare professionals, including physicians and therapists, to ensure comprehensive care.
  • Communicate effectively with patients, families, and caregivers to ensure understanding and compliance with care plans.
  • Document patient progress and care provided in a timely and accurate manner.
Requirements
  • Current LPN license in good standing.
  • 1 year of experience in an acute care or equivalent setting.
  • CPR certification with American Heart Association or American Red Cross.
  • Positive attitude and strong communication skills.
  • Dedication to quality patient care and a commitment to excellence.
