Key Responsibilities- Coordinate patient care services and ensure quality, cost-effective care for assigned patients.
- Manage patient treatment plans, collaborating with physicians, nurses, and other healthcare team members to achieve optimal patient outcomes.
- Conduct initial and ongoing assessments, develop disease management protocols, and monitor patient responses to interventions.
- Ensure continuity of care through discharge planning, resource utilization, and variance analysis.
- Function as a liaison between patients, families, healthcare team members, community resources, and employees.
- Graduate of a nationally accredited nursing program.
- Bachelor's degree in nursing.
- Nationally certified nursing professional.
- Bilingual English/Keres, Tewa, Tiwa, Towa, Zuni, or Navajo or bilingual English/Spanish.
- CPR for Healthcare/BLS Provider or Professional Rescuer certification within 30 days.
This is a full-time position with a competitive salary and benefits package. The ideal candidate will be able to work in a fast-paced environment and demonstrate excellent communication and interpersonal skills.
