Job Title: Site Lead Registered Nurse

At Cornell Scott Hill Health Corporation, we are seeking a highly skilled and experienced Site Lead Registered Nurse to join our team. The successful candidate will be responsible for guiding and supervising assigned nursing personnel in delivering patient care.

Responsibilities:
  • Conduct comprehensive assessments of patients to identify their needs and develop individualized care plans.
  • Collaborate with healthcare professionals to ensure seamless delivery of patient care.
  • Supervise and mentor nursing staff to ensure they are providing high-quality care.
  • Manage a selected caseload of patients and ensure their care needs are met.
  • Perform in-person and phone triage to assess patient needs and provide guidance to nursing staff.
  • Document patient information accurately and thoroughly in medical records.
  • Formulate patient-specific goal-directed plans of care that prioritize patient outcomes.
  • Implement care in a knowledgeable, skillful, and holistic manner, including follow-up with patients as needed.
  • Administer medications safely and effectively, ensuring patients' rights are respected.
  • Recruit, orient, and train new staff members, including LPNs and MAs, to ensure they are competent and confident in their roles.
  • Work with providers to assess the performance of LPNs and MAs.
Requirements:
  • Graduate from an accredited school of nursing and hold a current Registered Nurse license in Connecticut.
  • Minimum of one year of experience in a long-term care facility or rehabilitation center, with clinic experience highly desirable.
  • Excellent patient management skills and strong leadership qualities.
  • Ability to learn and utilize electronic medical records and other healthcare technology.
  • Knowledge of CLIA waived lab testing, urine dipsticks, and glucometers is necessary.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse groups.
  • Bilingual individuals are desirable, but not required.
