Key Responsibilities:

  • Assess patient needs and develop personalized care plans
  • Coordinate with interdisciplinary teams to ensure comprehensive care
  • Provide regular visits and updates to patients and families
  • Document patient records and progress notes
  • Supervise and support Home Health Aides as needed

Requirements:

  • Graduate from an accredited registered nursing program
  • Licensed as a registered nurse in the state of practice
  • Current CPR and First Aid Certification
  • Knowledge of quality assessment and assurance procedures
  • Valid driver's license and proof of insurance

Schedule: Monday-Friday, 8a-5p, with holiday on-call rotation
