VNA Health Group is seeking a skilled Physical Therapist to join our team in New Jersey. As a Home Health Physical Therapist, you will be responsible for providing high-quality physical therapy services to patients in their homes or alternate community settings.
Key Responsibilities- Evaluate and assess patients for physical therapy needs and develop personalized plans of action in collaboration with physicians.
- Document all changes in patient condition and update care plans and outcomes accordingly.
- Monitor quality and appropriateness of services provided.
- Adhere to agency infection control and safety policies.
- Adhere to the organization's policy regarding absenteeism and appearance.
- Physical Therapy Licensure in NJ required.
- ~2-3 years of prior clinical experience and community-based experience preferred.
- ~ Strong computer skills.
