About the Role

As a Social Services Assistant, you will play a vital role in promoting the well-being and quality of life for our patients/residents. You will work collaboratively with our interdisciplinary team to identify strengths, social, emotional, and mental health needs, and develop personalized plans to meet those needs.

Key Responsibilities
  • Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being.
  • Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs.
  • Hold meetings with patients/residents, families, and significant others as requested to provide support and information to improve quality of life.
  • Perform patient/resident discharge planning activities, including family/patient education, nursing center documentation, and referrals to appropriate agencies and suppliers.
  • Establish relationships and maintain contact and referral flow with community-based agencies/services that benefit our patient/resident population.

Qualifications

To be successful in this role, you will need:

  • A Bachelor's degree in social work or human services field.
  • Any certifications/licensures as required by the State of employment to practice in long-term care.
