Job SummaryThe Clinical Care Leader is responsible for utilizing the nursing process to provide individualized care to residents in the home setting. This includes assessment, diagnosis, outcomes/planning, implementation, and evaluation.
Key Responsibilities- Collaborate with inter-professional colleagues to plan, implement, and evaluate care
- Document resident history, symptoms, medications, and care provided in compliance with applicable standards and regulations
- Utilize technology to enhance quality resident care and collaborate with inter-professional team members
- Delegate to and supervise unlicensed personnel, assume overall nursing accountability for a resident assignment, and serve in leadership roles on department/organizational tasks forces and committees
- Bachelor's degree in nursing preferred
- Graduate from a nationally accredited nursing program preferred
- Minimum of two years' long term care or geriatric nursing experience preferred
- Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring
