Job Title: Registered Nurse

Mass General Brigham is seeking a skilled Registered Nurse to fill a challenging and rewarding role as a Nurse Navigator in our Home Health department.

Job Summary

The successful candidate will be responsible for identifying eligible patients and conducting clinical and social eligibility and safety screening while collaborating with the Home Hospital Admitting provider, the Home Hospital Case Manager, and the Administrative Coordinator.

Key Responsibilities
  • Attend daily huddles with the Home Hospital team to gather clinical information and identify patients based on catchment area and plan of care.
  • Interact with the Emergency Department interdisciplinary team and ED Care Facilitators to assist patient identification and facilitate successful transfers to MGB Home Hospital.
  • Collaborate closely with the MGB Home Hospital Admitting Provider to notify the Home Hospital Case Manager when perspective patients meet eligibility criteria.
  • Obtain consent from patients to transfer to MGB Home Hospital and communicate with the Administrative Coordinator and Administrative Manager regarding patient transfers.
  • Have real-time knowledge of capacity and capabilities of MGB Home Hospital and work with care areas to manage patient flow and placement to support Home Hospital transfers and census goals.
  • Utilize analytical and critical thinking in decision making and judgment that is aligned with organizational objectives.
  • Serve as a role model, coach, and facilitator, promoting thorough collaborative and respectful communication, and inspiring and encouraging teamwork.
Requirements
  • Current RN licensure in the state of Massachusetts.
  • Minimum 3 years of experience in emergency department, home hospital, or inpatient settings.
  • Varied experience preferred.
