Mass General Brigham is seeking a highly skilled Nurse Navigator to join our team in Brookline, Massachusetts.
Job Summary:
We are looking for a dedicated and organized RN to work as a Nurse Navigator for our Allogeneic Hematopoietic Cell Transplant program. The successful candidate will be responsible for guiding patients and their families through the complexities of the transplant journey, ensuring seamless coordination of care and optimal patient outcomes.
Key Responsibilities:
- Coordinate patient care and services, including pre-transplant work-up, medical clearance, and admission.
- Collaborate with the Transplant Search Coordinator to initiate Human Leukocyte Antigen (HLA) typing for recipients and potential donors.
- Work with the Transplant Search Coordinator and NMDP Case Manager to facilitate National Marrow Donor Program (NMDP) unrelated donor workups.
- Act as a primary point of contact for MGH clinicians, advanced practice providers, lab technologists, social work staff, and quality management.
- Maintain precise and organized records, and have a thorough understanding of sophisticated documentation and clinical testing required for allogeneic therapy and treatment plans.
Requirements:
- Current Registered Nurse licensure in the State of Massachusetts.
- At least two years of Hematopoietic Cell Therapy experience.
- Organized, with clear communication skills and ability to work collaboratively with the multi-disciplinary team.
