Job Summary:

The Nurse Navigator will work closely with patients, families, and healthcare teams to provide comprehensive care and support throughout the transplant journey. This role requires strong communication and organizational skills, as well as the ability to work autonomously and as part of a team.

Key Responsibilities:
  • Coordinate patient care and services, including scheduling, testing, and treatment plans.
  • Collaborate with healthcare teams to ensure seamless care transitions and effective communication.
  • Provide education and support to patients and families, addressing their physical, emotional, and spiritual needs.
  • Manage multiple electronic platforms and systems, including EPIC and NMDP.
  • Participate in interdisciplinary case review meetings and contribute to the development of care plans.
Requirements:
  • Current Registered Nurse licensure in the State of Massachusetts.
  • At least two years of experience in Hematopoietic Cell Therapy.
  • Excellent communication and organizational skills.
  • Ability to work collaboratively with job share partner.
  • Formal training and comprehensive understanding of EPIC scheduling and ordering systems.
