Patient Advocate Liaison

Conifer Health Solutions is seeking a highly skilled Patient Advocate Representative to join our team. As a key member of our patient advocacy team, you will play a critical role in ensuring that patients receive the best possible care and support throughout their hospital stay.
Key Responsibilities- Conduct thorough interviews with patients and their families to assess their financial needs and eligibility for various programs.
- Develop and maintain accurate and detailed documentation of patient interactions and financial clearance activities.
- Collaborate with hospital staff, including case management and social services, to ensure seamless communication and coordination of patient care.
- Utilize strong analytical and problem-solving skills to identify and resolve complex financial issues related to patient accounts.
- Provide exceptional customer service and support to patients and their families, ensuring a positive and empathetic experience throughout the hospital stay.
- High school diploma or equivalent required; associate's or bachelor's degree in a related field preferred.
- Minimum 2 years of experience in a patient advocacy or social services role, preferably in a hospital setting.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and hospital staff.
- Strong analytical and problem-solving skills, with the ability to navigate complex financial systems and resolve issues efficiently.
- Ability to work independently and as part of a team, with a strong commitment to providing exceptional patient care and support.
- Competitive hourly rate of $29.50, with shift differentials available for certain shifts.
