About the Role

We are seeking a highly skilled MDS Coordinator to join our team at August Healthcare at Wilmington. As a key member of our care team, you will play a critical role in ensuring the highest quality of care for our residents.

Key Responsibilities
  • Coordinate the completion of Minimum Data Sets (MDS) and associated processes to ensure accurate and timely submission of case mix data.
  • Chair care plan meetings and ensure that all necessary documentation is completed and up-to-date.
  • Monitor 24-hour condition reports for potential significant changes in resident condition and ensure that all necessary documentation is completed and up-to-date.
  • Review and submit case mix and PPS documentation to the state in a timely and accurate manner.
  • Conduct weekly Medicare meetings and review residents' continuing needs for SNF services and acuity level.
Requirements
  • 3 years previous experience in a nursing home with a strong focus in rehabilitative service programs preferred.
  • Must have an understanding of the RAI and PPS process with a demonstrated capacity for leadership.
  • Knowledge of clinical practices and procedures as well as laws, regulations and guidelines that pertain to long term care.
  • RN/LPN with current active licensure in the practicing State.
