Overview of Key Responsibilities:

The Patient Transition Specialist plays a crucial role within the sales team, ensuring a smooth transition for patients moving from a healthcare facility to a setting that addresses their post-acute care needs. This position requires effective communication and collaboration with both internal and external stakeholders to promote patient-centered care. The Specialist will evaluate patients to gauge their health literacy, identify the needs of patients and caregivers, and coordinate with referral sources and discharge providers to secure necessary documentation. It is essential to involve patients and their families in the care planning process.

Once the transition is successfully executed, the Specialist will liaise with the Agency to establish patient-centered plans aimed at achieving optimal health outcomes. The role demands strong communication and collaboration skills, enabling the Specialist to work effectively across various environments while utilizing clinical, sales, marketing, negotiation, problem-solving, and analytical abilities to drive market development initiatives and enhance patient care.

Qualifications:

Required Skills and Experience:

  • A minimum of three years of experience in a healthcare organization, preferably in home health or hospice settings.
  • Preferred qualifications include LVN, RN, PT, PTA, or COTA certifications.
  • CPR certification from the American Heart Association or American Red Cross is required for clinical roles.
  • Exceptional written, verbal, and listening communication skills.
  • Ability to manage conflicts, stress, and multiple tasks effectively and professionally.
  • Capability to work independently while fostering collaboration with team members, building relationships, and achieving results.
  • Strong understanding of customer and market dynamics, along with best practices in transitional care.
  • Excellent organizational skills, including time management and problem-solving abilities.
  • Familiarity with State, CMS, and accreditation survey processes.
  • Demonstrated customer relations skills and a commitment to delivering quality service.
  • Knowledge of healthcare laws, regulations, and various accreditation standards.
  • Proficiency in Microsoft Word, Excel, and Outlook, with a working knowledge of EMR databases.
  • Able to work with culturally diverse clients and address low literacy issues in care provision.
  • Commitment to uphold the core values and objectives of the Agency.
  • A valid driver's license, automobile liability insurance, and reliable transportation are required.
  • Willingness to travel between facilities, hospitals, and home care agencies, with travel expectations of approximately 75%-100%.

Educational Background:

A High School Diploma or GED is required. An Associate or Bachelor's degree in nursing or allied health is preferred.

Work Environment & Physical Demands:

The work environment is generally quiet but can be stressful due to deadlines and multiple tasks, alongside compliance with laws, rules, and regulations. The position requires visual acuity and dexterity, with the ability to sit, stand, and lift up to 25 pounds. Potential for auto-related incidents exists.
