Job Summary

A-Line Staffing Solutions is seeking a skilled Case Manager to join our team. As a Case Manager, you will play a critical role in coordinating care and services for our clients, ensuring they receive the support they need to achieve optimal health outcomes.

Key Responsibilities
  • Care Planning and Coordination: Develop and implement comprehensive care plans that address the unique needs and goals of each client.
  • Service Navigation: Connect clients with community resources, healthcare services, and other support systems to promote independence and well-being.
  • Communication and Collaboration: Work closely with healthcare providers, family members, and other stakeholders to ensure seamless care coordination and effective communication.
  • Risk Identification and Mitigation: Identify potential risks and barriers to care, and develop strategies to mitigate them and promote positive outcomes.
Requirements
  • Education and Experience: Bachelor's degree in a related field, such as social work, psychology, or healthcare administration, with at least 1 year of experience in case management or a related field.
  • Skills and Qualifications: Strong communication and interpersonal skills, ability to work independently and as part of a team, and proficiency in MS Office Suite applications.
