Role Responsibilities and QualificationsThe Clinical Nurse Case Manager is responsible for delivering high-quality, patient-focused care by overseeing rehabilitative services to achieve optimal patient outcomes. This role involves collaboration with the Rehabilitation Program Director and the Facility Case Management Director to ensure effective departmental operations.
Key Responsibilities:
- Provide orientation to patients, families, and caregivers regarding the case manager's role, rehabilitation philosophy, and discharge criteria, including insurance benefits and patient rights.
- Facilitate patient, family, and caregiver education, encouraging active participation in care discussions and planning.
- Ensure the execution of individualized treatment plans that align with the patient's strengths and preferences, promoting their involvement throughout the rehabilitation journey.
- Document findings from discharge planning evaluations and psychosocial assessments, communicating the needs and preferences of patients and families.
- Uphold a standard of exceptional patient interactions that reflect our commitment to outstanding service.
About Our Team:
- Collaborative teamwork across all levels of staff.

- Schedule: Weekend shifts from Saturday to Monday, 8 AM to 4:30 PM.
- Focus on adult patient populations.
Required Qualifications:
- Associate Degree in Nursing or Nursing Diploma is required.
- Bachelor's Degree in Nursing is preferred.
- Active Registered Nurse license in the state of practice.
- Minimum of 2 years of experience in case management or 3 years in clinical nursing is required.
- Experience with InterQual is preferred.
- Certification in Case Management is preferred.
