- A generous salary range of $110,000 - $140,000 per year, based on experience

The Role
As an Advanced Practitioner in Geriatric Care, you will be responsible for caring for patients, maintaining accurate and current patient records, and scheduling initial, urgent, and follow-up appointments as required. You will work as a team with our physicians, social workers, paramedics, and care team coordinators to deliver concierge care to every patient.
What to Expect
You will partner with other clinicians in the Care Community to manage the health and wellbeing of patients living in their homes. You will be responsible for engaging attributed patients into the WellBe program, providing primary care to patients in the home, and facilitating staff, patient, and family decision making through educational resources.
Requirements
To be successful in this role, you will require:
- Current licensure and certification as a Nurse Practitioner
- Experience working in geriatric care or a related field
- Excellent communication and interpersonal skills
- Able to work independently and as part of a team
- Ability to travel to patient locations
