Job Title: Case Manager

Join Royalty Staffing as a Case Manager and play a vital role in delivering high-quality healthcare services to our patients. As a Case Manager, you will be responsible for coordinating patient care, managing chronic conditions, and ensuring seamless transitions between healthcare settings.

Key Responsibilities:
  • Prescribe appropriate medication, health regimens, and lifestyle adjustments to promote patient recovery and prevent future health issues.
  • Manage stable patients with chronic conditions by utilizing internal resources and references to maintain their general health.
  • Triage and quickly recognize when treatment is appropriately provided on site versus in the urgent care or emergency room.
  • Respond to and provide crisis intervention services, ensuring behavioral health services are delivered in a culturally appropriate fashion.
  • Refer patients to the appropriate Behavioral Health providers, including psychiatrists, psychiatric PAs, and licensed MFTs.
  • Prioritize daily tasks and duties to maintain high-acuity patients out of emergency and urgent care facilities.
  • Complete all required medical documentation in the Electronic Medical Record in a timely manner.
  • Assist in developing and implementing procedures and protocols to facilitate the delivery of comprehensive health services.
Requirements:
  • Nurse Practitioner License or Licensed Independent Practitioner.
  • RN license and CPR Certification.
  • Demonstrated experience in community clinic and/or Managed Care System.
  • Experience working with the elderly.
  • Supervisory experience, including supervision of nurses.
Working Conditions:
  • Prolonged periods of standing and walking.
  • Frequent sitting.
  • Will be required to work at other sites.
  • Will be required to work on-call and participate in evening and/or weekend shifts.
