Job Summary:
The Physical Therapist delivers extensive rehabilitation services to patients referred by a physician or other qualified healthcare provider.

Key Responsibilities:
  • Upholds and advances the mission and values of Covenant Health Ministry.
  • Conducts assessments and therapeutic interventions under the direction of a physician or recognized healthcare provider while adhering to safety protocols.
  • Assesses patients according to departmental guidelines and formulates a therapeutic care plan that includes achievable long- and short-term objectives, service frequency, duration, and therapeutic techniques.
  • Engages with patients and primary caregivers to gather insights for the therapeutic care plan.
  • Integrates this feedback into therapy objectives.
  • Collaborates with the referring physician and other healthcare team members regarding the care plan and patient discharge requirements. Participates in relevant team meetings.

  • Tracks and assesses treatment outcomes, adjusting the treatment plan as necessary to meet established functional goals.
  • Records all evaluative and therapeutic interactions in the Electronic Medical Record in accordance with departmental protocols, ensuring clarity and accessibility for the care team.
  • Advises on and secures suitable assistive devices for patients, providing necessary training on their usage.
  • Offers educational resources to patients and/or caregivers, documenting the effectiveness of the instruction.
  • Appropriately refers patients to Physical Therapist Assistants and collaborates with other therapy professionals to ensure optimal patient care.
  • Understands and adheres to PPS, Medicare, and other reimbursement guidelines.
  • Specializes in orthopedic practice, working with surgical patients post-joint replacement and following the care standards set by the interdisciplinary team.
  • Maintains professional development through workshops and conferences relevant to Physical Therapy, sharing insights with staff post-attendance.
  • Ensures documentation of patient care aligns with reimbursement requirements.
  • Provides educational sessions on clinical knowledge and skills to nursing and other departments.
  • Actively contributes to the development of new programs and services within Physical Therapy.
  • Stays informed on professional matters through organizational memberships and literature, sharing relevant information with colleagues.
  • Attends departmental meetings and reviews minutes from any missed sessions.
  • Supervises assistant-level staff as appropriate, ensuring compliance with state regulations and professional standards.
  • Mentors new staff during their orientation, ensuring completion of the departmental orientation checklist and basic competencies.
  • Acts as a Clinical Instructor for students in affiliation with the department.
  • Completes operational duties in a timely manner.
  • Fulfills all operational documentation requirements, including daily charges and Medicare Certifications as needed.
  • Participates in departmental Quality Improvement initiatives by completing scheduled Chart Review Monitors.
  • Maintains cleanliness and organization within the department, adhering to infection control protocols.
  • Keeps all required licenses, certifications, and competencies current, completing annual compliance training on schedule.
  • Performs other duties consistent with this role.

Qualifications:
Knowledge and Skills:

Education and Experience:
  • Bachelor's degree required; Master's or Doctorate preferred.
  • Licensed as a Physical Therapist in the State of Maine.
  • Basic Life Support (BLS) certification is mandatory.

An equivalent combination of education and experience that demonstrates proficiency in the responsibilities outlined may be considered.

Working Conditions:
OSHA standards Category I, with potential exposure to blood/body fluids or tissue.

Must be capable of functioning in a fast-paced environment requiring multitasking, prioritization, and sound decision-making skills.

Exposure to various health risks, including disease and physical injury. Must possess the physical and mental capabilities to perform tasks associated with this role, which may involve prolonged periods of standing, walking, bending, and lifting up to 50 pounds.
