Long-Term Care Service Manager

As a pivotal member of our organization, you will play a crucial role in enhancing the health of our communities. Arizona Complete Health is dedicated to transforming healthcare for our members, focusing on personalized care and support.
Key Responsibilities:
1. Develop and assess comprehensive care management strategies to achieve optimal healthcare outcomes.
2. Create individualized service plans for long-term care members, educating them and their families about available services and benefits.
3. Evaluate member needs and available resources, facilitating plans that promote the best outcomes.
4. Collaborate with members, families, and care provider teams to ensure accessibility to essential services.
5. Provide resource support for various needs, including employment, housing, and independent living.
6. Monitor and adjust care plans based on ongoing assessments of member status and needs.
7. Interact with healthcare providers to ensure that member requirements are met effectively.
8. Maintain accurate documentation of member information and care management activities, ensuring compliance with regulatory standards.
9. Conduct home visits to assess member needs and collaborate with healthcare professionals.
10. Educate members and their families on healthcare procedures, service options, and benefits.
11. Offer feedback to leadership on potential improvements in care quality and service delivery.
12. Perform additional duties as assigned, adhering to all organizational policies and standards.
Qualifications:
Requires a Bachelor's degree along with 2 to 4 years of relevant experience, or equivalent experience demonstrating the necessary knowledge and skills.
Licensure:
A valid driver's license is required.
Compensation:
Competitive hourly rate of $46.68, along with a comprehensive benefits package that includes health insurance, 401K, stock purchase plans, tuition reimbursement, and paid time off. Total compensation may vary based on individual qualifications and experience.
