Key Responsibilities:
- Develop and implement comprehensive care management strategies to achieve optimal healthcare outcomes.
- Create or assist in the formulation of tailored service plans for long-term care members, providing education to members and their families regarding available services and benefits.
- Assess member needs and available resources, recommending and facilitating plans for the best possible outcomes.
- Collaborate with members, families, and care provider teams to ensure accessibility to necessary services.
- Offer resource support for various member needs, including employment, housing, and independent living.
- Monitor and evaluate care plans and member progress, making adjustments as necessary based on identified needs.
- Engage with long-term care healthcare providers to ensure that member requirements are met effectively.
- Maintain accurate documentation of member information and care management activities to comply with regulatory standards.
- Conduct home visits and other assessments to evaluate member needs and coordinate with healthcare providers.
- Provide educational resources to members and their families regarding healthcare procedures and benefits.
- Offer feedback to leadership on potential improvements in care quality and service delivery.
- Perform additional duties as assigned, adhering to all organizational policies and standards.
Qualifications:
- A Bachelor's degree is required, along with 2 to 4 years of relevant experience or equivalent expertise.
Licensure:
- A valid driver's license is necessary.
Compensation:
- The pay rate is $46.68 per hour. Arizona Complete Health provides a comprehensive benefits package, including competitive salaries, health insurance, retirement plans, tuition reimbursement, and flexible work arrangements. Actual compensation will be determined based on individual qualifications and experience.
