Healthcare Liaison Nurse

Job Summary

The Healthcare Liaison Nurse is a seasoned nursing professional dedicated to enhancing the quality of care for adult patients with benign hematology conditions. This role emphasizes a comprehensive, multidisciplinary approach to patient management, ensuring that each individual receives tailored, evidence-based care throughout their healthcare journey.


Key Responsibilities
  • Care Coordination: Oversee and promote comprehensive care for patients, conducting systematic assessments to develop personalized care plans. Identify and address barriers to effective treatment and coordinate necessary interventions.
  • Patient Education: Provide essential information and resources to patients and their families, facilitating a smooth transition from pediatric to adult care. Develop educational materials based on patient needs and promote awareness of research opportunities.
  • Clinical Support: Serve as a backup nurse when necessary, maintaining accurate clinical databases and ensuring compliance with quality standards.
  • Collaboration: Communicate effectively with a multidisciplinary team to ensure all aspects of patient care are addressed. Facilitate appointments and coordinate services across various healthcare disciplines.
  • Quality Improvement: Participate in initiatives aimed at enhancing patient care and satisfaction, contributing to ongoing quality assurance efforts.

Qualifications

Applicants must possess a Bachelor of Science in Nursing from an accredited institution, hold a valid nursing license, and have current certification in Basic Life Support (BLS).


Skills and Abilities

Successful candidates will demonstrate strong interpersonal and communication skills, attention to detail, and proficiency in relevant software applications.
