Job Title: Pharmaceutical Care Specialist

We are seeking a highly skilled Clinical Pharmacist to provide high-quality pharmaceutical care to our Military Service Members and their families at Matrix Providers in Newport, RI.

  • Employment Status: Full-Time
  • Compensation: This is an hourly position, paid bi-weekly. Estimated salary range: $120-$160 per hour.
  • Schedule: Monday - Friday

This role requires providing medication therapy management, conducting patient assessments, and collaborating with healthcare providers to optimize patient outcomes.

  • Requirements:

To be successful in this role, you must have:

  • A Doctor of Pharmacy (Ph.D.) degree from an accredited institution.
  • Completion of a clinical pharmacy residency or fellowship accredited by the American Society of Health-System Pharmacists or American College of Clinical Pharmacy.
  • At least one year of full-time experience as a clinical pharmacist in an ambulatory care setting.
  • A current, full, active, and unrestricted license to practice as a pharmacist.
Responsibilities:
  • Assess patient response to drug therapy and plan drug therapy based on physician-established diagnoses.
  • Order and evaluate laboratory tests necessary to evaluate drug therapy effects and outcomes.
  • Initiate, modify, or discontinue medications for ongoing therapy of chronic disease states in cooperation with the medical staff.
  • Monitor and manage pharmacotherapy requiring periodic adjustment due to specific or changing pharmacokinetic characteristics.
  • Administer prescription or non-prescription drugs according to established agreements or protocols.
  • Evaluate medical and medication histories for drug-related problems and adjust drug therapy accordingly.
  • Consult with other healthcare providers about patient treatment needs or options.
  • Perform a full range of pharmacist procedures.
  • Review patient profiles at the time of dispensing to monitor medication therapy closely.
  • Ensure prescription directions for use, dose, medication interactions, therapeutic duplications or overlaps, allergy information, age-specific dosage, and other pertinent data are appropriate for individual patients.
  • Provide medication and healthcare-related training to clinical staff on new medications, appropriate use of medications, and other topics as deemed appropriate by the department head.
  • Document significant medication interactions and pharmacy interactions with prescribers.
