Position Summary:

We are in search of a remarkable Rehabilitation Support Specialist who will play a crucial role in our mission. The successful candidate will assist individuals with disabilities in acquiring essential skills to enhance their ability to live, work, and engage in leisure activities according to their unique preferences.

The Rehabilitation Support Specialist will collaborate directly with individuals with disabilities to teach coping mechanisms and daily living skills. This role also involves working alongside employers and providing on-site support to facilitate the adjustment of individuals to their work environments and the responsibilities of maintaining employment.

Classification: Direct Care

Key Responsibilities:

  1. Design and participate in initiatives that foster recovery, rehabilitation, and overall wellness.
  2. Create and maintain written assessments regarding individuals' workplace adjustments and ensure proper documentation in case files.
  3. Identify challenges and implement strategies as per the individual treatment plans of program participants.
  4. Develop, oversee, and execute treatment planning objectives across all life domains.
  5. Observe and monitor individuals' adjustments through direct engagement and consultations with healthcare professionals, behavioral health specialists, employers, and other service providers.
  6. Contribute as a member of the multidisciplinary behavioral health team.
  7. Document interactions, challenges addressed, and strategies implemented with program participants.
  8. Ensure adherence to ongoing behavioral and physical health treatments.
  9. Assist in connecting program participants and their families with community resources and support services.
  10. Monitor individuals for indications of potential relapse.
  11. Support individuals in securing housing and other necessary resources for independence and self-sufficiency.
  12. Participate in case conferences or interdisciplinary team meetings, training sessions, and development workshops; document all relevant interactions and progress in the individual's case record.
  13. Provide vocational guidance and job coaching to individuals; conduct job/task analyses and develop job training plans.
  14. Oversee job placements with a retention goal of 90 to 180 days.
  15. Conduct site visits to job and internship locations and meet with individuals with disabilities as scheduled, including evenings and weekends.
  16. Deliver intensive skills training and employment adjustment counseling at the job site.
  17. Document all interactions and progress in the individual's case record.
  18. Perform additional duties as assigned within the scope of the role to further the mission, values, and vision of DLCP.

Qualifications:

  1. Associate's degree in a human services field.
  2. Certification as a Peer Recovery Specialist is required or must be obtained within 90 days of employment (candidates with this certification will be prioritized).
  3. Strong background in social work, behavioral health, addiction counseling, crisis management, mental health counseling, and motivational interviewing.
  4. Familiarity with sober-living home management practices and techniques.
  5. Ability to maintain organized records and communicate effectively, both verbally and in writing.
  6. Excellent time management skills with the ability to multitask and prioritize effectively.
  7. Attention to detail and strong problem-solving abilities.
  8. Strong organizational and planning skills.

Reports To: Director of Rehabilitation Services

Physical Requirements:

Must be capable of sitting and standing for 20 minutes and lifting at least 10 lbs.
