Job Summary

The MDS Department is responsible for developing and completing resident assessments in accordance with state and facility requirements. The MDS Coordinator will promote the physical and emotional well-being of residents, create and review resident care plans, and analyze financial coverage to ensure residents receive necessary care.

Responsibilities
  • Develop and implement resident care plans in accordance with Medicare requirements and ethical standards
  • Conduct initial and periodic interviews with residents and their families to gather information for resident assessments
  • Utilize the Resident Assessment Instrument (RAI) to gather information and create resident care plans
  • Collaborate with social services, dietitians, rehab specialists, and medical staff to implement and monitor resident care plans
  • Ensure all strategies comply with Medicare requirements and ethical standards
Requirements
  • Graduate of an accredited School of Nursing (RN)
  • Current/active license for Registered Nurse
  • MDS coordinator certification
  • Clinical experience in a nursing facility
  • Demonstrated clinical assessment skills
  • Knowledge of Medicare and Medicaid regulations and coverage guidelines as related to skilled nursing
