As a Transitional Care Navigator RN with Health First, you will play a vital role in promoting optimal clinical and resource outcomes for our patients. You will work collaboratively with the interdisciplinary team to ensure seamless transitions of care, providing education, support, and coordination to patients, caregivers, and staff members.
Key Responsibilities:
Quality/No Harm:
• Facilitate patient-centered transitional plans in collaboration with physicians, nurses, and the interdisciplinary team.
• Collaborate with the care transitions team to identify high-risk patients and ensure safe transitions to prevent readmissions.
• Apply process improvement methodologies to evaluate outcomes of care and identify areas for improvement.
Stewardship:
• Identify cost-saving options for patients and develop discharge plans that optimize resource utilization.
• Proactively identify and resolve system problems impeding diagnostic or treatment progress.
• Demonstrate understanding of level of care criteria and reimbursement factors for various care settings.
Requirements:
• RN licensure and experience in transitional care or a related field.
• Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
• Ability to think critically and problem-solve in a fast-paced environment.
