Job Summary

We are seeking a highly skilled MDS Coordinator LPN LVN LNAC to join our team. As a key member of our interdisciplinary team, you will play a critical role in facilitating the RAI Process, coordinating daily Case Management meetings, and ensuring timely MDS assessments. Your expertise in Medicare, Managed Care, and RAI Process will be invaluable in providing high-quality care to our patients and residents.

Responsibilities
  1. Facilitate the RAI Process under the direction of an R.N. or DNS.
  2. Work in collaboration with the interdisciplinary team to identify the needs of the patient/resident.
  3. Coordinate and conduct the daily Case Management meeting.
  4. Ensure timely MDS assessments according to state and federal regulations.
  5. Ensure interdisciplinary team completes designated sections of the MDS assessment.
  6. Ensure accurate coding of the MDS assessment with information obtained via medical record review, observation, and interview with center staff, patients, residents, and family members.
  7. Ensure documentation is available to support accurate coding of the MDS assessment.
  8. Maintain the tracking system of MDS assessment schedules (timeframes and due dates).
  9. Facilitate Care Plan conferences with the interdisciplinary team, patient, residents, and families.
  10. Obtain, review, and maintain all State and Federal reports, making appropriate corrections timely.
  11. Monitor Quality Measures and ensure that MDSs are accurate to support and reflect the Quality Measures.
  12. Provide education related to the RAI Process.
  13. Coordinate and complete electronic submission of required documentation to the State database and other entities per company guidelines and State and Federal regulations.
  14. Ensure Medicare and Medicaid regulatory guidelines are completed accurately and timely (i.e., certifications, denial letters, skilled documentation, coverage criteria, etc.).
  15. Provide Managed Care case management at the center level.
  16. Review Additional Documentation Requests (ADRs) with Health Information Management Coordinator (HIMC) to ensure all documents are available and meet the request.
  17. Participate in billing reviews (Triple Check) to ensure claims have supporting MDS assessment documents.
  18. Participate in Quality Assurance and Process Improvement Activities.
  19. Continues to update knowledge base related to Medicare, Managed Care, Medicaid, RAI Process, and computer technology. May be required to fulfill the job functions of a direct care nurse as determined by the needs of the patients/residents and center.
Qualifications
  1. Must hold current nursing license in the state of employment.
  2. Experience in Long Term Care.
  3. Computer skills.
  4. Knowledge of Medicare, Managed Care, and RAI Process Required.
  5. Participating in the LPN to RN Back to School transition program.
