Job Summary
We are seeking an experienced Licensed Practical Nurse (LPN) to join our team at HopeHealth, a leading provider of hospice and palliative care services. As a LPN, you will play a vital role in delivering high-quality patient care to individuals in their homes, including both pediatric and adult patients.
Key Responsibilities
- Provide direct patient care using advanced technology in collaboration with the patient's RN Case Manager and interdisciplinary team.
- Build strong relationships with patients and their families to make a meaningful difference in their lives.
- Communicate effectively with the care team to ensure seamless care coordination.
- Educate patients and caregivers on self-care techniques and provide support to promote independence.
- Follow physician orders and treatment plans to ensure optimal patient outcomes.
- Participate in an interdisciplinary team to deliver timely patient services and achieve positive clinical and quality outcomes.
- Accurately document patient care and data to inform care decisions.
Qualifications
- Graduate of an accredited nursing school with a current LPN license in RI and MA.
- Minimum of 1 year clinical experience with pediatric experience.
- Excellent clinical and communication skills, with the ability to work effectively in a team environment.
- Dual licensure in Rhode Island and Massachusetts required within 6 months.
- Experience in hospice or home care preferred, with a strong understanding of care coordination principles.
