Position Overview:
The Support and Service Coordinator plays a vital role in the CLTS Waiver Program, a Wisconsin initiative designed to provide Home and Community Based Services (HCBS) under Medicaid. This program aims to enhance the lives of eligible youth aged 0-22 with developmental, emotional, or physical challenges, ensuring they receive the necessary support for inclusion in their communities.
Key Responsibilities:
- Facilitate and coordinate services tailored to the unique developmental, emotional, and physical needs of enrolled youth.
- Engage in family-centered, trauma-informed, and culturally responsive service delivery through regular contact with families.
- Promote community inclusion and access for youth, ensuring they can thrive in their environments.
- Assist families in navigating program eligibility requirements and maintain ongoing assessments of the youth's health and safety.
- Identify at-risk youth and coordinate care with relevant agencies to provide comprehensive support.
- Respond to crises and critical incidents with effective intervention strategies.
- Act as a mandated reporter, ensuring the safety and well-being of youth by reporting concerns to appropriate authorities.
- Develop individualized service plans that align with the family's goals and needs.
- Authorize and manage the provision of services through the CLTS Program, ensuring quality and compliance.
- Advocate for families, empowering them to access resources and support independently.
- Provide transitional assistance during enrollment and disenrollment processes.
- Complete all necessary training and maintain accurate documentation of services provided.
- Adhere to policies and procedures set forth by the organization.
Additional Responsibilities:
- Participate in training sessions, team meetings, and case consultations as required.
- Support colleagues by providing coverage for families when necessary.
- Perform other related duties as assigned to fulfill the role effectively.
Qualifications:
Knowledge, Skills, and Abilities:
Demonstrated understanding of the needs of youth with disabilities; ability to manage challenging situations with professionalism; effective communication skills; proficiency in Microsoft Office applications; strong organizational skills; and cultural sensitivity.
Minimum Requirements:
- Bachelor's degree in a relevant field and a minimum of one year of experience working with individuals with disabilities.
- Completion of all mandatory training specific to the CLTS Program.
- Valid driver's license and reliable transportation.
- Ability to provide trauma-informed and culturally sensitive services.
- Commitment to maintaining professional relationships and adhering to ethical standards.
Work Environment:
This position is primarily office-based, with significant community outreach required. Flexibility in scheduling is essential, as evening and weekend hours may be necessary to meet the needs of families.
