Job Summary:

The MDS Coordinator will be responsible for ensuring timely and accurate completion of the RAI process and care management process from admission to discharge in accordance with company policy and procedures, and Federal, State and Certification guidelines.

Key Responsibilities:

  • Collaborate with the Interdisciplinary Team to assess resident needs and provide interdisciplinary schedules for MDS assessments and care plan reviews.
  • Ensure the Interdisciplinary team makes decisions regarding additional MDS assessments based on clinical criteria.
  • Coordinate and manage the daily stand-up meeting to review resident care and set assessment reference dates.
  • Comply with federal and state regulations regarding RAI process completion and coordination.
  • Monitor MDS and care plan documentation for all residents and ensure documentation is present in the medical record to support MDS coding.
  • Maintain current MDS status of assigned residents according to state and federal guidelines.
  • Enter resident information into computerized MDS programs and complete accurate coding of the MDS.

Requirements:

  • Graduate of an approved RN/LVN program and licensed in the state of practice.
  • Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
  • Excellent knowledge of Case-Mix, Federal Medicare PPS process, and Medicaid reimbursement.
  • Thorough understanding of the Quality Indicator process and OBRA regulations.
