Job Title: LPN - Homecare Specialist

We are seeking a skilled LPN to provide high-quality patient care in the comfort of their own homes. As a Homecare Specialist, you will work independently to assess, plan, and implement patient care plans, ensuring the best possible outcomes for our patients.

Responsibilities:
  • Utilize the nursing process to assess, plan, and implement patient care plans
  • Provide skilled nursing care and services in accordance with prescribed orders
  • Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition
  • Collect, analyze, and interpret data and information from healthcare members and document actual and/or potential nursing diagnoses
Qualifications:
  • Currently licensed as an LPN/LVN in the state in which the LPN/LVN will practice
  • Current TB or Chest X-Ray
  • Current BLS card
